PAW Financial Assistance Program
If you are a citizen of Yamhill County or the West Valley Area and are low-income or receive any type of government assistance, you might qualify for our PAW surgery prices.
Documentation of your qualifications must be provided to approve you to receive PAW Prices. Examples of government assistance programs that will qualify you:
- Oregon Health Plan
- Subsidized Housing
To apply for PAW financial assistance:
Call our Clinic Manager, Jennifer Choate, DVM, 971-267-2567. She will interview you on the phone and fill out the application during the phone call.
You will be asked to provide documentation of your financial status and/or participation in one of several government assistance programs (see above list). You may bring a copy of the documentation to the Spay/Neuter Clinic on a Tuesday, Wednesday, or Thursday, or send a picture of the documentation to Dr. Choate via text (971-167-2567) or email (email@example.com). You must provide documentation of assistance programs preferably before the day of the scheduled surgery. For low-income status, you need to provide family income and number of family members to qualify.
You may also print the PAW Pet Assistance Worksheet from this website or pick up a copy from the Spay/Neuter Clinic and complete the form yourself. You may bring the completed PAW financial assistance form and copies of your documentation of your financial status to the clinic on a Tuesday, Wednesday, or Thursday, or mail the form to Dr. Choate at 723 NE Evans St., McMinnville, OR 97128. Dr. Choate will review the forms and call you for an interview. You must complete the form, an interview, and the submission of documentation before the day of scheduled surgery to receive the PAW surgery price.
